Fatal accident casts shadow over progress in Equinor's safety performance

The frequency of serious incidents decreased in 2025, but the year was marked by a fatal occupational accident at the Mongstad refinery.

Equinor's safety results for 2025 show a generally positive development, but the year is also marked by a fatal accident at the company's refinery in Mongstad in September, where an employee died during a lifting operation. Equinor said in a press release.

- This is a tragic accident that has hit us hard. It must be safe to go to work for everyone who works for Equinor. We are working to continue learning from serious incidents like this, says Camilla Salthe, Group Director for Safety, Security and Sustainability at Equinor.

At the end of the fourth quarter of 2025, the serious incident rate per million working hours (SIF) was 0.21 against 0.3 at the end of 2024, which corresponds to an improvement of more than 30 percent. Serious injuries are included in the calculation of serious incidents. The serious injury rate per million working hours (TRIF) ended at 2.3 in 2025, which is unchanged compared to the previous year.

A total of six oil and gas leaks were registered in 2025 compared to seven in 2024, and according to the company, there have been no incidents with major accident potential during the year.

In 2025, Equinor had a high level of activity with many working hours performed at, among others, Mongstad, the Hammerfest LNG facility on Melkøya and the Empire Wind offshore wind project in the USA. At the same time, the company has had incidents and orders from the Danish Marine Industry Authority, which have been followed up with initiatives based on identified deviations.

Equinor states that it will continue its work to strengthen safety in 2026, building on the experiences from 2025 with a focus on technical and organizational improvements, culture and working environment, risk management and collaboration with suppliers.
